It encourages a commitment to behavioral changes that enables you to achieve a desired or ideal weight while maintaining healthy diet choices.

This fitness program coaches you toward making physical activity a priority in your life and fitting it into everyday activities. If you to improve your physical shape and consider yourself a “couch potato,” this program can teach you self-assessment and skill-building activities to help improve your personal level of fitness.
Short Circuiting Stresssm
This stress management program teaches you to manage stress by exercising, sleeping better, relaxing, breathing appropriately and working on other positive behavioral changes.
Cholesterol Countssm
This cholesterol management program concentrates on teaching you about cholesterol control — what it is, how it affects the body and health and what lifestyle choices are necessary to reduce and maintain healthy cholesterol levels.

This pre-diabetes program is designed to delay, if not prevent, the development of diabetes. It offers educational materials focused on preventing the onset of diabetes through diet, weight loss and exercise. Compliance with treatment plans and attendance of regularly scheduled physician appointments are encouraged.
Take The Pressure Offsm
This blood pressure management program educates about blood pressure, assists with behavior modifications for positive change and provides an overall understanding of the importance of blood pressure control and its affect on health.
Healthy Bonessm
This osteopenia program is designed to delay, if not prevent, the development of osteoporosis. It provides educational materials focused on preventing the onset of osteoporosis through good nutrition, nutritional supplements (calcium and vitamin D) and exercise. Health Coaches provide motivation and education on lifestyle changes that may prevent complications associated with developing osteoporosis.
Wise Passagesm
The Wise Passage menopause program offers personalized relief of menopause symptoms such as hot flashes, night sweats and mood swings through telephonic counseling with a Nurse Practitioner who is certified as a Menopause Clinician or Women’s Health Practitioner.
